The palliative consult team offers specialized palliative care consultation in the community or acute care for adults diagnosed with a life-limiting illness.
The consult team works in collaboration with the patient’s current Most Responsible Provider (physician or nurse practitioner), in a specialized and consultative role. The team is consulted for complex symptoms and advanced care planning related to life-limiting/threatening illness. Support includes making recommendations for care options.
