Hospice care is a specialized service that provides 24/7 care to individuals who are approaching end-of-life and their families. Services are available for both adult and pediatric populations and may include respite.
The care teams focus on your comfort and quality of life and can support you and your family throughout the last months, weeks, or days of life.
Services are provided by a specialized interprofessional palliative care team that includes (but is not limited to): doctors, registered nurses, licensed practical nurses, social workers, and healthcare aides.
Availability of other professionals such as spiritual counsellors and other services such as volunteers may be different for each hospice.

The North Zone has one designated publicly funded 10 bed hospice which is located in Grande Prairie. Admission requires a physician’s referral and eligibility criteria must be met.
Depending on where you are located within the North Zone, there are other Palliative and End-of-Life Care spaces located in varies care settings.

Service Information
For individuals living in the community, your physician or nurse practitioner will need to make a referral on your behalf to a palliative consult team. Once received, a consultant will assess your care needs and review your wishes with you and your family.
If you are a patient in a hospital, your attending care team will make a referral to a palliative consultant. The consultant will visit you on your unit to assess your needs.
Service Information
Hospice beds in Central Zone are geographically dispersed. Admission to these beds can be assessed through the local home care offices. Any questions about admission, eligibility and process can be taken to your local home care office, case manager or Continuing Care Access Centre.
Resources
The Calgary Zone PEOLC program delivers residential hospice care in partnership with the contracted providers. Referrals to hospice can only be made by a Palliative Home Care Case Manager or a Palliative Care Consultant. If you think hospice may be right for you or your loved one, talk to your physician or Home Care Case Manager about a Palliative Care Consult.
Service Information
The Palliative Care Teams in the South Zone deliver residential hospice/palliative care at St. Joseph’s in Medicine Hat and St. Michael’s in Lethbridge. Admission to hospice or palliative care units is coordinated by the Palliative Care Team physicians. If you think hospice or the palliative care unit may be right for you or your loved one, talk to your physician or Home Care Case Manager about a consult to the Palliative Care Team.
