In keeping with the 2021-2024 AHS Virtual Health Strategy, the DON SCN supports the long-term, consistent use of virtual health technologies and delivery of high quality virtual care in both community-based and specialty-care chronic disease management (CDM) programs, including delivery of diabetes, obesity and nutrition services.

In collaboration with partners from the Alberta Healthy Living Program, the DON SCN has:

• evaluated patient experience and outcomes
• supported program navigation
• contributed to development of tools, resources and best practices to enable provision of appropriate, safe, high-quality care that is accessible and consistent
