Perioperative glycemic management pathway for reducing length of stay in Albertan surgical patients (supporting the Surgery SCN)

  • In Alberta, 35-40% of patients have high blood sugars after surgery
  • Of these, 20% have diabetes, 10% do not, and 5-10% may have undiagnosed diabetes
  • Patients with high blood sugars after surgery have worse outcomes than patients with normal blood sugars
  • This pathway improves patient screening and helps manage patients at greatest risk of high blood sugar following surgery to improve outcomes

Empowering food-insecure Albertans to manage their diabetes through a subsidized healthy food prescription program

  • Food insecurity can be life threatening for people with type 2 diabetes because they need to eat healthy foods to control their blood sugars
  • A subsidized healthy food prescription program can help patients afford to eat a healthy diet, improving their health and reducing their need for healthcare services
  • This project studies the reach, effectiveness, and implementation of this type of program in primary care clinics in Alberta
