How to Apply

To apply to the Public Health Dental Clinics, dental care for Albertans, you will need:

  1. Public Health Dental Income Assessment Form
  2. A form to confirm that you don't have a dental insurance plan
  3. Evidence of income by providing the clinic with your Notice of Assessment from Revenue Canada. The Notice of Assessment is the letter sent from Revenue Canada confirming your income tax information. If you do not have your Notice of Assessment, you can phone 1-800-959-8281 and request that an Option C printout be sent to you.
  4. If you are experiencing financial difficulties and your income for the latest tax year was above our low-income cutoff, fill out the Temporary Eligibility Assessment for Reduced Fee Dental Care.
