The following tool is for the purposes of reporting behaviour within the Department of Pediatrics, both positive and negative. The Department of Pediatrics values workplace safety and takes the content of submitted reports seriously. Unless otherwise specified, all responses will be kept confidential. Input will be received by the Deputy Head, Professional Affairs through the following generic email pedsmdconcerns@ahs.ca and may be discussed with other Department leadership, as appropriate. Anonymized data will be saved for tracking within the Department. Please note, if you would like a formal review of this reported event, please complete the form below including your name for submission to the Associate Chief Medical Officer, Physician Health, Diversity & Wellness as identification is required for this to proceed as per AHS Medical Staff Bylaws. Confidentiality will be maintained throughout the process, in accordance with AHS bylaws and will be collected as per Section33(c) of the FOIP Act.
