The provincial Guideline for Patient’s Death in the Home Care Setting outlines a standard approach and practice for healthcare providers that leads to improved quality of care for patients and families during an already difficult time. The guideline provides direction for our regulated and unregulated Home Care providers on how to prepare for and/or approach an expected or unexpected death of a Home Care patient. The scope of this work includes all ages and focuses on Home Care patients residing in either a private home or seniors’ lodge setting. The online toolkit includes links and resources that support the education and implementation of the guideline.
