Upload your digital immunization records, including your QR code, if available. Your actual vaccine record is required for upload. The uploaded records MUST contain first name, last name, date of birth, vaccine name and date of immunization. Handwritten records are not accepted.
Any Albertan who has received a COVID-19 immunization outside of the province, or outside of the country, can upload their immunizations for review and verification, to then be added to MyHealth Records.
Out-of-province and out-of-country immunization records can be submitted on behalf of yourself or your child / youth under 18 years.
If you have been immunized in Alberta through an AHS clinic, pharmacy, or physician office, you DO NOT need to upload any immunization records.
All immunizations administered in Alberta will be automatically updated on your MyHealth Records account within 2-3 weeks following your immunization.
Submitting your out-of-province or out-of-country immunization records ensures your health record is fully up-to-date and provides an accurate record of who has been immunized.
Your records will be reviewed by AHS. Verified submission information will display in your MyHealth Records account. It may take 2-3 weeks for your information to show.
