Any Albertan can submit immunization records, including out of province/country records, on behalf of themselves or their child/youth under 18 years.
Please only submit immunization records for clients who meet one or more of the following criteria:
Your official immunization record is required for upload. The uploaded records MUST contain first name, last name, date of birth, vaccine name(s) and date of immunization(s).
For COVID-19 immunization records, please submit your digital record including the QR code if available.
If you have been recently immunized in Alberta through an AHS clinic, pharmacy, or physician office, you DO NOT need to upload any immunization records.
All immunizations administered in Alberta will be updated on your MyHealth Records account within 2-3 weeks following your immunization.
If your Alberta administered routine immunization is not showing in MyHealth Records, please contact your immunization provider or MyHealth Records Help.
For missing COVID-19 immunization records please contact the COVID Records Helpdesk.

If you are unable to access the online form, a copy of your immunization record can also be brought to an AHS Public Health Clinic.
