Access & Disclosure / Health Information Management

Releases individually identifiable health information to patients, authorized representatives, service providers and other third parties in accordance with the Health Information Act.

In accordance with Alberta's Health Information Act, this is a service which enables the release of individually identifiable health information with or without the consent of an individual.

Alberta Health Services (Hospital-inpatient / outpatient) does not maintain records for private physicians, medical clinics, or walk-in centres, please contact them directly to attempt to obtain a copy of their records.
Healthcare providers can submit a request for Alberta Health Services records by contacting our office via fax or email.
