The Program offers home-based support to families with children that have developmental delays or disabilities, aged 0-5 years of age.The program offers: information about development, behavior, play and parenting connections to community resources and services support to your family setting goals and planning for the future collaboration with your child’s health team services that are provided in various ways; in your home, in the community, virtual appointments or phone consultations
