Thrombosis Central Access and Triage

Central intake for all acute thrombosis. Associated clinics provide assessment, treatment, and education for people with acute, newly diagnosed blood clots.

  • client teaching and follow-up
  • investigation and treatment of disease and related conditions
  • short term anticoagulation medicine (e.g., warfarin, heparin) management - stabilization
  • consultations
  • follow-up time ranges from 3 months to 1 year
