Personalize Care in the Community

Staff support a therapeutic group home supporting youth with CFS status and extreme mental health challenges to live in the community and transition to a lower level of care and increased independence.

Personalized Community Care is a therapeutic group home providing placement, treatment and transitional support to youth with extreme mental health challenges. The multi agency partnership is with Child and Family Services, Closer to Home and AHS.
