COPD Self Initiative Team Clinic

The COPD Diagnostic clinic is meant to provide access to services that will provide Education on COPD prevention, treatment and self management strategies and optimize patient quality of life.

The COPD CSI Team clinic services may be used for patients:

  • for follow up of COPD
  • new diagnosis of COPD
  • smoking cessation
  • COPD education
  • medication education
  • nutrition education

Patients with the following are excluded:

  • less than 17 years of age
  • with physical or mental limitations that prevent them from participating in the self management program
  • with advanced lung cancer (metastatic, palliative chemo or radiation)
  • involved with palliative care
  • currently admitted as an inpatient
  • who does not have the ability to be contacted

Referrals are accepted 24/7 however, referrals are only triaged during clinic hours Monday thru Friday

Service is provided to 17 years of age and older

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Referrals are ONLY accepted from the COPD Diagnostic Clinic or follow up of previously assessed patients of the COPD Self Initiative Team or Self Initiative Team Respirologist.
