Stroke Outpatient Rehabilitation Team

Provides coordinated outpatient stroke rehabilitation services via a specialized interdisciplinary team that includes Physiotherapy, Occupational Therapy, and Speech Language Pathology.

Clients must be physically and cognitively able to participate in an active rehabilitation program:

  • will participate in either a high intensity program of 2-3 attendances per week for up to 12 consecutive weeks, or a low intensity program of 1 attendance per week for up to 6 weeks
  • must agree to attend the recommended therapy sessions per week per discipline involved
  • should expect their progress in the program to be reviewed at regular intervals to ensure they are meeting program expectations

Eligibility

Clients must have a primary diagnosis of stroke/CVA as per a physician:

  • must have identified functional goals that are appropriate for an ambulatory community-based rehabilitation program
  • must be referred by a Physician, Allied Health Discipline, Home Care Services, Inpatient Stroke Units, Unit 35 Rehab, Early Supported Discharge (ESD) Program
  • referral will be placed in chronological order on a waitlist based on date received
