Diabetes Management

Provides education and support for adults with diabetes or pre-diabetes.

Offers education and support for people who have diabetes or are at risk of developing diabetes, including:

  • staying well
  • information on diabetes / pre-diabetes
  • how to manage diabetes

Group classes and individual appointments are offered on:

  • type 1 diabetes
  • type 2 diabetes
  • gestational diabetes (and pre-gestational diabetes)
  • being high-risk for diabetes

In person or virtual (telephone or Zoom) appointments are available.

You can refer yourself or be referred by a doctor or healthcare provider. Review of your current health information such as A1C, blood glucose levels, cholesterol and any other relevant results will occur, along with education and lifestyle change conversation.
