Patients with persisting or worsening symptoms since discharge or completion of therapy.

Patients MUST:

• have a documented prior COVID infection or be deemed by two independent physicians / NPs to have had acute COVID-19 infection in the absence of any other obvious diagnosis
• be at least 12 weeks post COVID diagnosis
• must be 18 years or older

Plus a minimum of ONE of the following:

• emergence of new symptoms such as autonomic, respiratory, cardiac, endocrine, neurological, etc. for >12 weeks not attributed to another chronic condition
• persistent functional challenges including brain fog, fatigue, mobility concerns, mood / anxiety for >12 weeks; Post COVID Functional Scale (PCFS) score of 3 or 4
• complex multi-system concerns impacting social safety net, ability to self-manage, or lack of financial resources since COVID diagnosis
• required hospitalization, multiple subsequent visits to the ED / hospital, or family physician due to ongoing symptoms of COVID

Referrals will be accepted from the Central Zone (Red Deer and south ONLY), Calgary Zone and South Zone.
