Home Rehabilitation Team

Provides short-term, client-driven, home-based rehabilitation intervention.

The Home Rehabilitation Team provides an in-home rehabilitation service for adults with a recent decline in independence, function and/or mobility living within the Medicine Hat or Lethbridge areas.

Service Providers May Include
occupational therapists (OTs), physiotherapists (PTs), recreation therapists, registered nurses (RNs), social workers, speech-language pathologists (SLPs), therapy assistants
Eligibility

Eligibility includes:

  • 18 years of age and older
  • must be aware and agree with the referral
  • must be medically stable
  • Must require involvement of at least 2 disciplines (OT, SLP, PT, RN, Rec, SW)
  • Must have recent decline (3 months) in independence, function or mobility. May be at risk of needing placement in a facility or increased level of care but formal placement process has not been initiated/completed
  • Client can safely manage in their home environment (with or without family/other informal support)
  • Cognitively able to participate (i.e. demonstrates carry over from session to session)
  • Motivated & able to participate for 60-90 minutes/day (up to 30 minutes of activity), up to 5 days/week and up to 12 weeks duration
Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information. Self referrals are accepted but will require the client to provide demographic and medical information over the phone (Meet & Greet process).


Wait Times

An estimated wait time will be provided at the time of appointment booking. Generally, wait time is under 5 days.

Fees

Medical equipment/products may be prescribed - inclusive of AADL (Alberta Aids to Daily Living) benefits. The program itself is funded through Alberta Health.
