Active Rehabilitation Unit

A referral-based inpatient rehabilitation unit. The goal is to assist clients to become stronger so they may regain independence.

This service provides care to people who have had significant medical events that have lead to deconditioning, which may include:

  • have had surgery
  • have had an illness
  • have been in a hospital for a long time and need to get their strength back
  • are having frequent falls

Service Providers May Include
health care aids (HCAs), licensed practical nurses (LPNs), occupational therapists (OTs), registered nurses (RNs), recreation therapists, rehabilitation therapist assistants
Eligibility

Eligibility includes:

  • medically stable
  • has functional goals that can be improved with rehabilitation
  • does not have an infection that would greatly restrict their ability to participate in the rehab program due to required precautions
  • patient's behavior is appropriate and non-aggressive
  • patient has agreed to referral
