Coordinated Intake - Children's Rehabilitation Services

 Location: Wetaskiwin and Area

Social Workers complete an initial needs assessment for children and youth with developmental delays and match them with therapy services offered by our team.

Social Workers review and complete an initial intake for children with diagnosed or suspected developmental delays. The Social Worker will connect the child with the appropriate Therapists on the Children’s Rehabilitation Services Team including Speech Language Pathologists, Physical Therapists, Occupational Therapists and other providers. The Social Worker may refer the client to other community based services or other health providers as needed.

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Anyone can refer.

If a child requires services from an Occupational Therapists (OTs), Physical Therapists (PTs), and Speech Language Pathologists (SLPs), an intake interview must be completed over the telephone with a Social Worker.
