Location: Aberhart Centre
Provides teaching and information to pediatric patients with Down syndrome, their families, or healthcare providers about resources and recommended health surveillance according to the American Academy of Pediatrics (AAP) guidelines. We also do consultative and / or primary care for children 0 - 18 with Down syndrome.The Clinic Coordinator will act as a link between families and community programs to help coordinate care, and offers: prenatal consultation education sessions for caregivers, families and health professionals resources and information about community supports and services for families research and tools about Down Syndrome for doctors and other health professionals referrals or consultation on a case-by-case basis for assessment, management, and education

Must have confirmed diagnosis of T21 and be between 0 - 18 year of age.

Can be referred by parent alone.
