Anticoagulation Clinic

Provides anticoagulation services for people with a variety of health issues.

Services include:

  • treatment for blood clots
  • treatment dose anticoagulation initiation and dosing (atrial fibrillation, heart valves)
  • anticoagulation for complex health issues (pregnancy, cancer, etc.)
  • anticoagulation management for clients with unstable INRs
  • anticoagulation bridging for surgery or invasive procedures (warfarin interruption, and coverage with low molecular-weight heparin)
  • resources and education for patients, families, and health providers

Eligibility

Patients must have a family doctor who is able and willing to take part in the person’s care and management and to resume care when the person is discharged from the clinic.
