Provides anticoagulation services for people with a variety of health issues.Services include: treatment for blood clots treatment dose anticoagulation initiation and dosing (atrial fibrillation, heart valves) anticoagulation for complex health issues (pregnancy, cancer, etc.) anticoagulation management for clients with unstable INRs anticoagulation bridging for surgery or invasive procedures (warfarin interruption, and coverage with low molecular-weight heparin) resources and education for patients, families, and health providers

Patients must have a family doctor who is able and willing to take part in the person’s care and management and to resume care when the person is discharged from the clinic.
