Anticoagulation Management Services

Clinic staffed by pharmacists and nurses. Provides initiation of anticoagulation therapy and longterm monitoring of patients with high thrombotic or bleeding risk.

Services include:

  • coordinating laboratory testing
  • dosing decisions for anticoagulation medicine
  • education

The service is in communication with the patient’s primary care doctor.

Eligibility

Calgary Zone adult patients who require support for anticoagulation management which would include: warfarin initiation, low molecular weight heparin initiation or peri-procedural management, chronic anticoagulation with labile INR in high risk clotting or bleeding patient or initiation of DOAC in high risk bleeding or clotting patient.

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Must be referred by Calgary zone Physician or Nurse Practitioner.
