Geriatrics - Inpatient Rehabilitation

The inpatient Geriatric Rehabilitation Program offers assessment and treatment to older adults with medical problems to assist them in being able to go back to living in their community.

This service provides:

  • rehabilitation therapy
  • medicine management
  • nursing care
  • social work services

Service Providers May Include
audiologists, dietitians, doctors, feeding and swallowing therapists, health care aides (HCAs), licensed practical nurses (LPNs), occupational therapists (OTs), pharmacists, physical therapists (PTs), psychiatrists, psychologists, recreation therapists (RTs), registered nurses (RNs), speech language pathologists (SLPs)
Eligibility

The service is targeted to older adults who have experienced acute physical, cognitive and social decline in the past two to three months.

Clients need to be:

  • able and motivated to take part in active rehabilitation (e.g. be physically able to do the activities)
  • medically stable and want to return to their community
