Geriatrics and Continuing Care South Zone

Provides services to support people age 65 and older with mental health and physical health concerns including a geriatric assessment / rehabilitation unit, a day hospital, and a community outreach team.

With a goal to optimize healthy community living for seniors, this service provides:

  • Geriatric Assessment Unit (GAU) includes: assessment, treatment, and/or support re-entering the community (as needed) or placement in continuing care at the level the person needs.
  • Seniors Health Day Program offers: assessment, treatment and rehabilitation for people who are having health issues related to getting older but do not require long term care placement or hospital admission. This day hospital program does not provide crisis intervention.
  • Geriatric Assessment Team: Assess and treat - Clinic visit or home visit
