Continuing Care Access

Offers telephone information, screening and referrals for individuals who require access to Continuing Care services including Home & Community Care, Continuing Care Home Type A and Type B, Palliative Care, Adult Day Programs / CHOICE, and Children's Home Care.

Provides a central telephone number and assistance for people who require access to Continuing Care services in the Edmonton area, including:

  • Home & Community Care
  • Children's Home Care
  • Day Programs / CHOICE (Comprehensive Home Option of Integrated Care for the Elderly)
  • Alberta Aids to Daily Living (equipment surg supplies)
  • Access to Continuing Care Home Type B and Type B Secure (formerly DSL and DSL4D)
  • Access to Continuing Care Home Type A (formerly LTC)
  • Edmonton Zone Palliative Care Program

Regular business hours are Monday - Friday 7:00 AM - 4:00 PM

After hours support is available from 4:00 PM - 11:45 PM Monday to Friday, and Saturday and Sunday 7:00 AM - 11:45 PM for those individuals already on a Continuing Care Program.

Eligibility

Anyone living in Alberta with a valid provincial health-care card (or registration in progress) can be assessed for Continuing Care services. Each client will be individually assessed.

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

Consent, Alberta Health Care number and current demographic information is required.

Call-backs are made between the hours of 07:00AM and 10:30PM - Please ensure unavailable times are specified during initial call.


Wait Times

Preliminary information is initially received by administrative support staff. Return call are based on priority and call volumes.
