CHOICE Restorative

CHOICE South Restorative is a short-term, intensive rehabilitation program that partners with clients to improve their health and independence. This program is guided by a "work with you" approach, where clients actively participate and work towards their functional goals.

Clients will come to the CHOICE South Restorative Program several times a week. Based on their identified goals and care plan, they can receive the following:

  • medical monitoring and treatment
  • rehabilitation or restorative therapy
  • therapeutic recreation, cognitive and social programs
  • personal assistance with daily tasks like going to the washroom (as needed)
  • health and wellness education
  • meals and snacks

Access
Service Providers May Include
home support workers, licensed practical nurses (LPNs), nurse practitioner, occupational therapists (OTs), physiotherapists (PTs), recreational therapists, registered nurses (RNs), social workers, therapy assistants
Eligibility
  • Adults primarily 55+
  • Live in a home or lodge in the community
  • Have a valid Alberta health care card
  • Clients who are medically stable and have an achievable functional goal that will promote independent living in the community
  • Clients who require treatment from two or more regulated health professionals
  • Clients who are able and in agreement to attend and actively participate in individual and/or group programs for up to 6 hours per day
  • able to attend multiple days per week as needed based on assessments

Wait Times

Waitlist will be determined by available capacity at the program and number and urgency of referrals. Waitlist is reviewed on a regular basis.

Fees

All personal care costs, including professional support, are funded by AHS and provided by the contracted Operator.
