For the 2017-2018 Influenza Immunization Program all community providers are required to complete the Influenza Program Registration Form. This registration form provides the basic information needed to provide your site with influenza vaccine. Providers are asked to complete this form in order for the Alberta Health Services (AHS) Zone Contact to obtain accurate accounting of the number of partners participating in the influenza immunization program.
Confirmation of program enrolment will occur when you receive the 2017-2018 Medical Officer of Health Influenza Immunization Program Letter. This letter will outline all aspects of the influenza immunization program for the 2017-2018 season.
Vaccine will be ordered through the Alberta Vaccine Inventory (AVI) system for all Zones. Information on ordering will be communicated directly to you, through your Zone contact, by the end of September. You must submit the signed Registration Form before vaccine will be released to community offices/clinics/sites through AVI.
Pharmacies must order and receive Influenza Vaccine through their primary pharmacy wholesale distributors.
This is a reminder to all vaccine providers involved in the provincially funded immunization program. All vaccine storage and handling requirements outlined in the Alberta Health Vaccine Cold Chain Policy must be fully implemented in order to receive provincially funded vaccine. Ability to order vaccine will be impacted for sites that are not compliant with the policy.
