Alberta Healthy Living Program - Central Access and Triage

This central access referral service provides a single point of entry for patients who are living with a chronic condition or chronic pain that have been referred by a health care provider or wish to self-refer to our health education classes, self-management workshops, nutrition counselling or supervised exercise.

This is the Central Triage Department for the Alberta Healthy Living Program (AHLP) in the Calgary Zone. Referrals that meet criteria will be processed and patient's will be scheduled into the referred sevice.

All patients must have at least one chronic condition to be eligible for our program.

The Alberta Healthy Living Program offers health education classes, self-management workshops, nutrition counselling for adults living in the community or living with a chronic health condition.

Adults living with a nutrition-related health condition are eligible for nutrition counselling with a registered dietitian. Participants will be asked to attend a relevant Health Education class, if available, before booking a nutrition counselling appointment.

Eligibility

Eligibility criteria differs between services.

Service Access

Healthcare providers should consult the Alberta Referral Directory for service referral information.

This service is internet based and telephone information, all services are offered via Zoom or by phone. Registration is required and by appointment only.

AHLP also offers in-person programming for Supervised Exercise Classes at limited locations.

Health Care Provider referrals as well as patient self-referrals for Nutrition Counseling Services or Group Exercise Classes are accepted.

Referrals for Self-Management Workshops and Group Health Education Classes are not required.

Patients may self-refer by going to our website and completing the online registration form or calling in to our call centre.
